At the top are the letters ISZ 3 MES which I presume are two monograms with the initials for whom it was first intended.
Under the initials are two hearts chained together but separate, out of which a plant is growing from each. Next to each heart is a little padlock.
Under the padlock, a skull and under the skull are two crossed skeleton keys.
On either side of the intersected skeleton keys is a bird holding a ring in its beak.

- symbolism -
In heraldry, the hearts symbolize love, charity and sincerity and the chains binding the hearts together often symbolize “separated but not disunited.”
The plants growing from the hearts might symbolize fertility.
The skull is a symbol of mortality... it's a reminder that life is short.
The key is symbolic of authority and sentiment, since love has the power of unlocking and opening up the heart.
The lock, or padlock symbolizes guardianship and dominion, security and protection.
I believe the two birds are love birds holding rings in their beaks.
